You can leave your name or not. A prayer request needs no name attached to it, so the form does not ask for one.
If you are in danger, or thinking about hurting yourself, please stop reading and call or text 988 (the Suicide and Crisis Lifeline, in the United States, free and around the clock). Outside the US, call your local emergency number.
Please do that rather than writing here. The people who answer are trained for exactly this, and they are awake right now.
Spiritual direction is an ongoing relationship, and it belongs with a priest, who can also hear your confession.
Ask at your parish office. Every parish either has someone or knows where to send you, and it is an ordinary request rather than a strange one. That will serve you better than anything I could offer.
If you are a student of mine, come and find me at school, or go to campus ministry. This arrangement protects you. Please use the ordinary channels, and do use them.
